Hospice & Community Care is the largest nonprofit hospice and palliative care provider in Pennsylvania. Last year, we provided hospice and palliative care and bereavement support to over 1,900 people in York County.
Our Sunflower Pediatric Care offers specialized care and comfort designed around the unique needs of children with complex, serious illnesses and their families. This includes perinatal specialized care for parents anticipating the birth of a baby with a life-threatening condition.
The most common diagnoses for the children in our care is cancer, genetic/congenital and neuromuscular. Children like baby Arlo, a 4-month-old with a life-limiting genetic condition, or Nathan, an 18-year-old college student with a rapidly progressive brain tumor. Though Arlo’s and Nathan’s medical situations were vastly different, they had more in common than not. They were, and are, beloved, with families struggling in unimaginable ways to cope with the reality of losing a precious child. They both thrived in the comfort and security of their caregivers’ presence and at home, with familiar surroundings and routines.
When a child or teen is diagnosed with a life-threatening illness, the associated physical, emotional, and spiritual challenges can be overwhelming. Hospice & Community Care’s Sunflower Pediatric Team provides children from prenatal to young adults and their families with personalized care and support, with the goal of managing symptoms and coordinating care with their other healthcare providers so that the families can remain together in their home. The whole family is cared for with both palliative – when treatment is being sought – and hospice care – when end of life is imminent, as well as bereavement support for as long as they need it.
Community support is needed to ensure all families caring for a child with serious illness can receive the care and support they most desperately need. Insurance does not cover palliative care for children, and many of our families needing hospice care are uninsured or underinsured.
Thank you for helping us care for our most vulnerable children in York County.
To provide personalized, compassionate care and support for patients and families coping with serious illness through the end of life.
Our care focuses on:
• Enhancing a patient’s quality of life by reducing physical, emotional, and spiritual pain and distress.
• Providing compassionate psychosocial and spiritual support for the entire family.
• Partnering with our patients, their caregivers and family, and medical specialists to ensure seamless communication and care coordination; and
• Facilitating opportunities for families to create meaningful memories.
