Hospice & Community Care provides personalized care and support to patients and families in York County through hospice and palliative care and bereavement support. Hospice & Community Care's team of physicians, nurse practitioners, nurses, aides, social workers and chaplains help patients and families live life to the fullest. Care is provided to patients at home, in personal care facilities, at local hospitals, and the Bob Fryer & Family Inpatient Center.

Our name says it all, “Hospice & Community Care.” We are more than just a hospice organization, we provide community care. We are always looking for ways that we can better serve everyone, from early diagnosis of a serious illness through end of life to bereavement support. The more that we can address unmet needs in our community, the more that we can ensure that everyone has access to the personalized care and comfort they need.

Hospice & Community Care has been providing care and support to patients and families since 1980 through hospice, palliative and supportive care and bereavement support. Hospice meets patients and families where they are to walk with them in their journey, regardless of ability to pay.
